- Full time position, we are looking for 5 days a week of coverage but are flexible and can take a provider who can only offer 4 days a week as well. NOTE: We may be able to get them started sooner if a clean file and is eligible for temps but we won't know more until we get the provider application.
- Shift times are 9am-5pm (the last appointment is typically at 4:30pm). Provider will have a break in the middle of the day.
Rates:
Reg: $500 Hr
Guarantee of 8 hours per day
OT to apply after 40 hours per week
Infusion space is only 4 chairs , small service line.
Combination of hem/onc and oncology, 20% hematology (people with anemia).
Sole provider practice, there will be resources from the other clinic.
- Radiation oncologist onsite, 3 days a week. Putting a new radiation machine, but not preforming at the site.
Patient Count is 18 (16 are follow up, 2 new consults)
OTHER DETAILS:
- Preferred BC, we can consider someone truly BE.
- Provider is going to be working in a medical group clinic but it is a hospital licensed space which will mean they need to obtain privileges for Santa Rosa Memorial Hospital. They will not do any work in the hospital. The infusion space is within the clinic.
- CA license and CA DEA required.
-BLS required.
Please note that the privileging timeline will not begin until the RFA and application are turned back in. Typical turn around for RFA is 48-72 hours. Application should be submitted within 5 days.
o Typical privileging timeline is 90 days.
o Once the file is completed, we can look at temporary privilege eligibility.
o All specialties require 2 years of current Hospital clinical activity in the specialty.
o Gaps greater than 3 months must be explained.
o Include current number of case logs upon presentation for surgical specialties.
