Job Summary

The HEDIS/Risk Outreach Coordinator is responsible for assisting with the deployment and implementation of various Stars and Risk adjustment programs tied to medical records retrieval strategies, collecting and tracking medical records from offices and internal systems within the organization. The coordinator will also work closely with Stars, Risk, UM/CM, Provider Relations, Member Services and other Clever teams to schedule appointments for members and for scheduling education or retrieval visits with providers, which could be IPA, MSOs, Physician Offices. The coordinator will also be involved with continuous training and education on HEDIS, Stars and Risk Adjustment measures to close gaps in care.

Functions & Job Responsibilities

  • Candidate will work with the Stars and Risk team to collect member records and conduct reviews of these records by contacting providers and placing the results collected into a specific database.
  • Facilitate fax or telephone call notification for providers utilizing reports.
  • Create PowerPoint decks and other training materials for providers and staff.
  • Collects and compiles the required documentation to maintain critical quality improvement functions.

Telephonic and electronic outreach to patients

    • HIPAA compliant outreaches to patients for Medication adherence/utilization and other risk/quality gaps closure support including but not limited to CAHPS and HEDIS/Risk gaps.
    • Conducts member outreach for quality initiatives including HEDIS or wellness care gaps, medication adherence, member experience, Health Risk assessments, and other quality related outreach campaigns.
    • May also be required to conducts member outreach from provider office as needed.
    • Warm transfer of any calls to the vendors per the need of the project.
    • Identifying barriers that prevent patients from closing any gaps and work with care team members to help patients be successful in managing their health.
    • Coordinate with members of the patient care team, including vendors, providers, pharmacists, nurses, and patient care coordinators to address patient needs.
  • Other duties as assigned.

Qualifications

Education and Experience:

  • EMR experience is a plus, must be okay with travel to provider offices.
  • Bachelors degree or minimum two (2) years' experience in health plan, managed care or equivalent experience.
  • Knowledge of CMS Star Programs, HEDIS, and Risk Adjustment are a plus.
  • Valid driver's license, reliable transportation, and insurance required with clean driving record.

Skills:

  • Prior experience with project management/coordination and timelines
  • Demonstrated effective organizational skills.

Harvest Truck & Trailer Driver

The Harvest Truck & Trailer Driver is responsible for safely transporting harvested product from field locations to the Distribution Center (Cooler) while maintaining full compliance with company, safety, and DOT regulations. This position plays a critical role in supporting daily harvest operations and requires strong communication, attention to detail, and the ability to work independently in a fast-paced, time-sensitive environment. Drivers must be adaptable to changing field conditions, shifting schedules, and seasonal travel. The role may require long or irregular hours based on harvest demands.

Areas of Responsibility:

  • Transport harvested product from field locations to the Distribution Center (Cooler) in a safe, timely, and efficient manner.
  • Operate trucks and trailers in compliance with DOT regulations, company policies, and safety standards.
  • Perform required pre-trip, en-route, and post-trip vehicle inspections; report any mechanical or safety issues immediately.
  • Accurately complete all paperwork related to product transport, equipment use, and daily logs.
  • Maintain ongoing communication with Commodity Managers, Supervisors, Forepersons, and Transportation Logistics Coordinators to ensure smooth harvest flow.
  • Complete required documentation at the Carton Yard, including load verification and equipment tracking.
  • Maintain certification to move harvest equipment during the day as needed to support production continuity.
  • Assist with equipment positioning, trailer staging, and other operational needs to support harvest efficiency.
  • Follow all company safety protocols, including PPE requirements, safe driving practices, and field-specific procedures.

Preferred Qualifications:

  • Valid Class A Commercial Driver's License (CDL) with Doubles and Triples endorsement.
  • Drivers' motor vehicle records, safety records, and drug and alcohol violations history checks required.
  • Self-motivated and able to work independently with minimal supervision.
  • Flexible to work in any assigned area.
  • Strong attention to detail and commitment to safety.
  • Availability to work at least six days per week.

Background: Cahaba Medical Care Foundation is a community health center organization providing medical, pharmacy, dental, and behavioral health services to diverse underserved communities in Jefferson, Bibb, Perry, and Chilton counties. We are a Level 3 Patient-Centered Medical Home and Joint Commission accredited organization, committed to increasing integration and coordination of behavioral health and primary care. This is an exciting, fast paced practice with a strong mission and commitment to providing high quality care.


Position Summary

This position requires a mission-driven and detail-oriented Advanced Practice Provider (APP) to serve as our lead clinician for Medicare Annual Wellness Visits (AWVs), preventive care services, and related quality initiatives. This is a high-impact position designed to help our organization improve preventive care rates and quality outcomes across all 28 Cahaba Medical Care clinics.

The ideal candidate will have a passion for health promotion, quality improvement, and rural health equity. This provider will perform in-person wellness visits across our locations, follow up on necessary documentation (including insurer-specific requirements), and support training for other staff and providers in performing high-quality, compliant AWVs. This position reports directly to the Chief Executive Officer (CEO), Chief Medical Officer (CMO), and works in close coordination with the Quality Improvement Officer.

Cahaba Medical Care expects all of our licensed providers to host learners from time to time, since we are a teaching institution. Responsibilities & Duties

  • Perform in-person Medicare Annual Wellness Visits and other preventive care visits across Cahaba Medical Care sites.

  • Ensure all required elements of AWVs are accurately completed, documented, and submitted per payer and CMS guidelines.

  • Submit insurer-specific forms and documentation in payer portals (e.g., closing care gaps, diagnosis attestation, HCC coding updates, etc.) to ensure loop closure.

  • Collaborate with the Quality Team to ensure visits meet internal quality standards without additional remediation.

  • Serve as a clinical resource and subject matter expert for AWVs, helping to train Medical Assistants, front-desk staff, and fellow providers on workflow best practices.

  • Identify system barriers to completion of AWVs and work collaboratively to improve clinic workflows and efficiency.

  • Support Cahaba Medical Care’s system-wide goal of achieving a defined percentage of Medicare AWV completion prior to the calendar year’s end (goal to be determined annually).

  • Provide occasional care for acute, walk-in visits if clinically appropriate and needed on-site.

  • Participate in organizational quality and training initiatives, special projects, and data-driven improvement work related to population health.

  • Familiarity with payer-specific documentation or HCC coding workflows.

  • Training or teaching experience (e.g., MA or provider education) strongly valued.

  • Spanish language proficiency is a plus but not required.


Qualifications:

  • Certified Nurse Practitioner or Physician Assistant with active Alabama license.

  • At least 2 years of experience preferred, particularly in outpatient primary care, geriatrics, or quality improvement.

  • Strong understanding of Medicare Annual Wellness Visit components and CMS documentation requirements.

  • Exceptional attention to detail and follow-through.

  • Willingness and ability to travel to multiple clinic locations across Central Alabama (reliable transportation required).

  • Demonstrated commitment to underserved populations and mission-driven care.

  • Comfortable working independently while coordinating with a multidisciplinary team.


Location

  • Six-County Region (including Bibb, Perry, Chilton, Jefferson, Dallas, and Wilcox Counties)

  • Travel is required with mileage reimbursement. This position involves driving to multiple Cahaba Medical Care sites across Central Alabama.

  • Assists clients with accessing and maintaining appropriate housing.
  • Attends to the safety, health, and well being of members.
  • Is personally responsible and held accountable for work hours and time management as coordinated with Program Director or designee.
  • Provides after hours, “on-call” coverage on a rotational basis based on program needs.

Qualifications:

A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from high school or an accredited GED program and. working at least two (2) but less than four (4) years of full-time equivalent experience providing direct mental health services.

Lived experience is a plus.

BASIC PURPOSE
The purpose of this position is to assist in providing 24/7 program monitoring services for all clients at the various Social Services sites as assigned. Program Attendants will be assigned to work either at the Center of Hope, Residence Building, Transitional Living Center and/or parking lot areas to keep the program sites and their participants safe. This position plays a pivotal role in controlling our buildings environments, ensuring that our clients and staff are safe from disorderly behavior and/or pests. This position will be responsible for controlling what items and individuals are granted entry into TSA buildings.

ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Answer office telephone in a courteous and professional manner.
2. Issue and initial receipts for donations received (Dollar amount only included if donation is monetary).
3. Report problems in writing to Director.
4. Maintain strict confidentiality of clients’ information.
5. Ensure all residents abide by house rules and enforce curfew hours and house schedule.
6. Maintain an updated time log of all activities, periodically make bed checks, monitor the facility, and conduct spot checks.
7. Place business calls for clients who do not have phone access.
8. Monitor client’s participation, activity and volunteer work in house and record this information on the master roster.
9. Prior to wake-up call, make rounds outside the office, dorms, and parking lot to prevent possible theft of company property as well as inspecting for anything out of the ordinary.
10. Make necessary announcements in regard to programs, counseling, services available, meals, etc.
11. Contact the proper authorities in the event of an emergency; the point of contact these situations.
12. Keep work area clean and orderly while maintaining a professional attitude at all times.
13. Clean windows, floors, restroom in common areas: lobby, office, cafeteria and hallway.
14. Keep hallways and doorways clear at all times.
15. Secure building after curfew and make irregular, frequent rounds throughout the remainder of shift to assure adequate security.
16. Inspect grounds periodically for cleanliness and safety.
17. Work with Director to ensure compliance to various contracts with local area hospitals, private organizations and governments.
18. Accomplish various tasks and responsibilities as assigned by various site supervisors. Additional tasks are to be in line with general duties and responsibilities.

KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS REQUIREMENTS
1. High School Diploma or GED equivalent.
2. Ability to work effectively with a diverse population.

PHYSICAL REQUIREMENTS:
1. Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis
