Job Summary and Qualifications

The Registered Nurse coordinates and delivers high quality, patient-centered care in accordance with the nature and specific requirements of the department, and consistent with the scope and standards of practice for the relevant state and specialty. In collaboration with medical providers and other members of the care team, the RN provides individualized, comprehensive, and compassionate care using established nursing models such as “Assess, Perform, Teach, and Manage.” The RN serves as an advocate for patients/families/caregivers and models a commitment to the organization’s vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.

  • Performs procedures, monitoring, or other functions as ordered by the medical provider(s). Documents the administration of care in the patient medical record in a timely and thorough manner.
  • Teaches patients/families/caregivers about patient medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete understanding.
  • Teaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes. Prepares patients and families/caregivers for future self-management.
  • Manages evidence-based practices and continuous process improvement initiatives to meet and exceed quality measures and enhance the exceptional care provided to patients.
  • Manages the quality of care by collaborating with a multidisciplinary team to coordinate, evaluate, customize, and deliver a holistic patient treatment plan that promotes and supports recovery.
What qualifications you will need:


  • Advanced Cardiac Life Spt must be obtained within 6 months of employment start date
  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • NIH Stroke Scale must be obtained within 6 months of employment start date

1. Admits/readmits patients and provides intermittent nursing service to patients and their families in compliance with agency policies and State and Federal Regulations, incorporating the following:

  • A complete physical and psychosocial assessment.

  • Evaluation of patient safety on a 24-hour basis.

2.  Establishes and maintains case mix weight, utilization, patient satisfaction that aligns with industry standards and agency operational objectives for quality care.

  • OASIS documentation correlates with the Plan of Care.
  • Uses recommended Oasis assessment techniques 100% of the time.
  • OASIS, documentation, and plan of care correlate to 95%.

3.   Documents all services provided in the clinical record in accordance with State and Federal Regulations and Agency policy:

  • Documentation, including activities and mileage is exported within 36 hours of visit or prior to next scheduled visit.
  • Obtains physician verbal/written orders and complies with all reporting requirements.
  • Participates in coordination of care with physicians and with other disciplines as appropriate.

4.   Manages work assignment in effective and efficient manner:

    • Visits all new therapeutic clients within 24 hours of referral unless otherwise specified
    • by the physician.
    • Reports any cases to the Supervisor of Clinical Services where goals can not be met
    • or at-risk situations exist and responds appropriately.
    • Remains flexible in daily planning, allowing time for emergencies or unexpected
    • visits.
    • Maintains productivity as established by the Agency.
    • Utilizes payer sources correctly, reflecting changing levels of care.
    • Delegates patient care responsibilities to Home Care Nurse (LPN) as appropriate.
    • Demonstrates an understanding of the principles of case management.
    • Demonstrates ability to manage cost and quality

5.   Establishes and evaluates the Plan of Care to be delegated to the Home Health Aide, orients and supervises the Home Health Aide in accordance with State and Federal Regulations and Agency policy:

    • Develops written plan of care for the Home Health Aide and updates this plan as patient’s condition warrants; plan of care must be reviewed every 60 days.
    • Obtains input from Home Health Aide regarding patient progress.
    • Prepares written performance evaluations of home care aides under his/her supervision.
    • Provides supervision of home care aides at a minimum of every 14 days or as required by payer source.

6.   Develops and maintains sufficient professional skills and credentials to meet the needs of patient care:

  • Attends a minimum of 12 hours of in-service per year to include mandated annual in-services.
  • Participates actively in self-appraisal and the formation of a plan of self-development.
  • Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement.
  • Seeks appropriate clinical input to ensure appropriate patient care.
  • Projects professional image through attitude and compliance with dress code.
  • Capable of precepting new nurses and students.
  • Functions as a team member with consideration to all staff.
  • Provides Agency with current copy of professional license.
  • Maintains CPR certification.

Ancillary Duties

1.   Performs other duties as necessary to support Agency programs as assigned by the Supervisor of Clinical Services.

    • Agency committees as requested.
    • Flu/special clinics as needed.
    • Elder Wellness Clinics as assigned.
    • Special Agency or community programs during and/or after regular working hours.

2.    Performs other duties as necessary to maintain current Agency personnel policies:

    • Gives adequate time and attention to administrative duties.
    • Abides by Agency weekend/holiday rotation of nurses.

Job Summary:

·         The Patient Care Administrative Nurse Supervisor is an experienced Registered Nurse and is responsible for supervising and monitoring the Nursing department(s) on an assigned shift in accordance with nursing standards and practices as well as current Federal, State and local standards, guidelines and regulations.

·         This position provides leadership for multiple hospital departments in the absence of the unit manager, director, or administration.

·         This position ensures the appropriate delivery of direct and indirect patient care via the utilization of the Nursing Process and creates a positive environment for the professional practice of nursing.

·         The role provided leadership to ensure continuous improvement within quality, safety and in alignment with Balanced Scorecard, key priorities and strategic plan

·         Directs and implements evidenced based strategies to transform care delivery and attain optimal outcomes

·         In partnership, implements, facilitates, measures, evaluates clinical quality initiatives including both process and outcome measures

·         Analyzes data and partners to compose actionable process improvement, status reports and data driven presentations to share with both leaders and staff

  • Collaborates with the Clinical Logistics Center to open isolation beds, to expedite and prioritize rooms, ensuring there are limited blocked beds, working with units to ensure samples are collected and resulted in a timely manner to maximize capacity, manage 1:1 patient observation resources and communicate potential downgrades in the ICUs and goals of care changes.

You would be joining the Ayer Neuroscience Institute at Hartford Hospital in the position of Nurse Manager on the Bliss 9i which is16 bed unit composed of a combination Neuroscience and Medical ICU.  This role has the benefits of being part of the Neuroscience Institute along with the resources and teamwork that it brings.  Your role, in partnership with the Nurse Director, would be to support the management, budgeting, staffing and patient care this combined ICU. Our work centers around a shared governance philosophy and our home grown nursing professional practice model serves as a framework by which nursing care is delivered. By practicing within this model, we ensure that our practice is consistent across all specialties and that goals are clearly defined. The care that we provide is centered on enhancing the human health experience and supported by the following elements of nursing that align with our leadership behaviors; science, ethics, advocacy and art.

We provide care to patients with a wide range of complex, highly acute medical/neuro issues utilizing a variety of advanced technologies and devices including CRRT, monitoring intracranial pressures, brain tissue oxygenation and hemo-dynamic monitoring. Our patient population include but are not limited to; respiratory failure, gastrointestinal bleeding, liver injury/failure, kidney injury/failure, sepsis, drug/alcohol withdrawal, stroke, head/spinal cord trauma and brain hemorrhage. RNs in this specialty also must have a family-centered approach because of the critical nature of our patients. We also work closely with our Critical Care Clinicians, Neuro-intensivist, Physician Assistants and Advanced Practice Registered Nurses to provide the highest quality of care for our patients.

Functioning within the framework for professional nursing practice, the Patient Unit Manager supports and assists the Director. This position has 24 hour accountability for managing resources (human, material and financial), to ensure quality, cost-effective delivery of patient services for specific patient populations, in a defined area across the continuum. This is accomplished by direct action, delegation, coaching and support of unit-based shared governance councils. Reports to Director of Nursing for the Ayer Neuroscience Institute.

  • Bachelor's degree required.* Master's degree preferred. One degree must be in Nursing.

Nurse Practitioners and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.


Qualifications


Requirements:

·         Licensure: Unrestricted State of Connecticut Medical License (or eligible)

·         Training: Nurse Practitioner (Family, Adult/Geri Nurse Practitioner) or Physician Assistant

·         Experience: 2+ years’ experience in a primary care or relevant outpatient setting

The Adult Primary Care (APC) clinic is located at 132 Jefferson St., Hartford Hospital. APC Clinic has over 9,000 patient visits annually with an average of 40-50 patients per day. We are a busy outpatient ambulatory care clinic setting at a teaching hospital. We see patients for preventative annual care as well as patients with a wide range of health problems such as diabetes, high cholesterol, asthma/COPD and hypertension. Staffing includes attendings, APRNs, Residents, Psychologist, Social Worker, Nutritionist, Pharm D, RNs, MAs and MAAs. APC facilitates an ongoing relationship between patients and clinicians and fosters participation by patients in decision making about their health and their own care. The majority of our patients are bilingual with Spanish being their primary language and have health coverage under Medicaid. APC provides opportunities for disease prevention and health promotion as well as early detection of problems. APC guides patients through the health system, including appropriate referrals for services to other health professionals. We are recognized as a Patient-Centered Medical Home.

 

Job Summary:

The Asst Nurse Manager Amb is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. This position supervises a work group to ensure that a high quality, cost-effective patient and family centered care is provided in support of the managed area’s strategic plan, and the function's goals and objectives.

This position works collaboratively with functional leadership and is responsible for the oversight of daily operations of the assigned area including the supervision of nursing and non-licensed staff. This position manages resources, quality improvement, payroll, creating and maintaining colleague schedules and monitors patient throughput to facilitate efficient clinical operations. In addition, this position is responsible for a safe and therapeutic unit environment, which includes working as staff RN to assist as needed.

This position supports leadership in ensuring standards of excellence and professional development are fostered and met. This position also participates in and may lead Performance Improvement work in their assigned area.

Job Responsibilities:

  • Oversees all activities of the assigned practice/clinic - provides leadership and direction to direct reports and providers, and acts as positive role model.

  • Drives the development and execution of short and long-term business plans by collaborating with physician leadership and system administration in strategic planning and marketing for the practice/clinic.

  • Identifies and implements ways to maximize efficiency, productivity and resource allocation.

  • Resolves issues and notifies leadership of situations that may hinder team's ability to provide excellent service.

  • Responds promptly and effectively to patient and provider concerns.

  • Analyzes data reports and acts on significant trends affecting overall effectiveness of the department.

  • Participates in inter-departmental meetings and serves as liaison to outside stakeholders.

  • Develops, implements, monitors and documents policies, procedures, and protocols that ensure compliance with all internal policies and external regulations.

  • Provides office-based nursing care in collaboration with provider as needed.

  • Calls in prescription medications to pharmacies under direction of provider, and observes and documents patient’s response to pertinent medications.

  • Manages and develops colleagues to include performance feedback and reviews, promotions, salary recommendations, corrective action and separations. Teaches, coaches and mentors colleagues to encourage their professional growth and development.

     

  • Manages staffing schedule and payroll to include monitoring and addressing attendance issues.

     

  • Assists nursing leadership with investigating and reviewing patient concerns, key performance indicators (KPI), colleague health exposures and compliance issues.

  • Communicates department/leadership decisions and policies to colleagues. Cascades the flow of information from leadership to the clinical team

  • Maintains active involvement with Clinical Quality Improvement activities.

  • Participates on committees as requested.

  • Performs other related responsibilities as required.

     

 


Qualifications


  • Bachelor of Science in Nursing. Master’s degree preferred.

  • Current CT RN license
  • Existing incumbents as of March 1, 2024, must obtain BSN within 3 years of this date.
  • 2 years of nursing experience required. At least one year of supervisory experience preferred.
  • Certified in Basic Life Support (BLS) provider credentialed form the American Heart Association (AHA).

  • Other certifications may be required by specialty.
