Qualifications:

Examine, diagnose, and treat assigned acutely ill hospitalized patients at Community First Medical Center. Manage all assigned patients through the continuum of hospital care. Follow assigned patients upon admission to the intensive care unit throughout their hospital stay. Coordinate post-acute care for all assigned patients. Leadership of Rapid Response/Code Blue events and collaboration with APPs and the ED as needed for procedural support.

24/7 hospitalist model with continuous in-house APP coverage and hospitalist oversight.


ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Diagnose, manage the clinical issues, deliver and coordinate comprehensive medical care to acutely ill hospitalized patients

2. Order treatment regimens and tests, and interpret results of tests such as laboratory and radiology

3. Prescribe medications as appropriate and as required

4. Conduct patient contact/rounding/care management that actively encourages and includes professional input from all members of the patient care team

5. Consult with specialty and primary care physicians where appropriate

6. Communicate with CFMC interdisciplinary care team, family members and others upon admission, when treatment plans change, and at discharge to maintain continuity and quality of care

7. Conduct discharge planning and assure timely discharge of patients

8. Assure accurate and timely completion of all clinical and professional records and documentation, so as comply with local, state and federal regulatory, licensure and accreditation requirements

9. Responsibile for APP supervision and participation in code/RRT management.

10. Collaborate with other CFMC employees and physicians to create a positive and engaging work environment

11. Demonstrate strong written and verbal communications skills

12. Expectations around interdisciplinary communication, throughput, and LOS improvement.


SUPERVISORY RESPONSIBILITIES

Can supervise Nurse Practitioners and Physician Assistants.


WORK SCHEDULE

7-on/7-off rotation, 12-hour shifts, and separate coverage for AM, PM, and Weekend roles per ED’s posting plan.

Education and/or Experience:

1. Position requires an MD, DO, and completion of advanced graduate medical education in Internal Medicine, with board eligibility or board certification in Internal Medicine. Family practice trained physicians with previous hospitalist experience may be considered.

2. Training experiences as mandated by the American Board of Internal Medicine for board eligibility are required.

3. Previous experience and demonstrated procedural skills in intubations and central lines; ability to lead Code Blue required.

Head of Programmatic Media & Audience TargetingAbout Property LeadsProperty Leads is revolutionizing the real estate investing industry by delivering high-quality, motivated seller leads to real estate professionals.We're a fast-moving, data-driven team that values results, ownership, and efficiency over bureaucracy.Role OverviewWe are seeking a strategic and highly technical Head of Programmatic Media & Audience Targeting to own and scale our programmatic marketing efforts.You'll lead our investment across DSPs and leverage CMP / DMP platforms to build precise audiences of homeowners looking to sell.This is a high-impact, senior-level role for someone with deep experience building successful programmatic campaigns using 1st-, 2nd-, and 3rd-party data.You will have end-to-end ownership of campaign strategy, execution, and optimization--without needing to manage a large team.If you've built profitable paid media funnels at scale and thrive in a lean, performance-obsessed startup environment, this role is for you.Key ResponsibilitiesChannel Strategy & ExecutionOwn the strategy, setup, and scaling of programmatic media buying across platforms like The Trade Desk, DV360, and other DSPs.Build and manage targeting strategies using CMPs / DMPs (e.g., LiveRamp, Lotame, Segment) to find homeowners ready to sell.Develop and iterate high-performing funnel structures and campaigns optimized for lead volume and ROI.Audience Data & TargetingDesign and activate audience segments using first-party CRM data, lookalike models, and third-party intent datasets.Collaborate with data teams to onboard and refresh audience cohorts across platforms while ensuring data compliance.Leverage behavioral, demographic, and geographic signals to isolate high-converting seller profiles.Performance Optimization & ReportingMonitor campaign performance in real time to hit CPL, ROAS, and quality goals.Build and manage dashboards to track performance across creatives, audiences, placements, and geos.Conduct rigorous A / B testing on messaging, frequency, landing pages, and retargeting logic to improve results.Cross-Functional CollaborationPartner with growth, sales, and engineering to optimize the full lead lifecycle from ad click to conversion.Provide feedback loops from performance data to creative, messaging, and broader marketing strategy.What We're Looking For5-10years experience in performance marketing with a focus on programmatic media buying and audience-based targeting.Demonstrated success driving scalable, profitable campaigns using DSPs and data onboarding tools.Deep knowledge of campaign architecture, bid strategies, lookalike modeling, and media attribution.Experience targeting homeowners or audiences in high-intent verticals (real estate, mortgage, insurance, home services, etc.).Ability to work autonomously, execute rapidly, and continuously optimize with a testing mindset.Bonus SkillsHands-on experience with platforms like The Trade Desk, DV360, LiveRamp, Neustar, Lotame, BlueKai, or Segment.Familiarity with GA4, Looker, or similar BI / analytics tools.Prior experience in lead generation funnels, paid search, or conversion rate optimization.Exposure to real estate, insurance, or home services performance marketing.Why Join Property Leads?100% remoteHigh-growth company with major upside and direct impactResults-driven culture--no micromanagement, just ownershipOpportunity to build and lead a critical performance functionWork with a collaborative, motivated team solving meaningful problems in real estate investingJ-18808-Ljbffr.
