Position Highlights & Primary Responsibilities

The salary range for this role is $226,000 to $366,000 annually based on full-time employment. Salary range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. About This Role

As a Primary Care Physician (PCP) at Seen Health, you will be part of an interdisciplinary team providing holistic, participant-centered care for frail and elderly adults within the PACE program. You will provide direct primary and preventive care to participants and work collaboratively with your team to manage the complex medical, social, and functional needs of an underserved senior population. As a physician entrepreneur, you will have the unique opportunity to cultivate high-performing clinical talent, redefine traditionally fragmented senior care and caregiver experiences, and collaborate with our technology and operations teams to ensure the scalable delivery of culturally-sensitive, highest-quality care to underserved populations.

Responsibilities

  • Provide comprehensive, culturally-sensitive primary care to PACE participants, focusing on the prevention, diagnosis, and treatment of common and chronic medical conditions affecting older adults.
  • Conduct initial, periodic, and annual health assessments and develop individualized care plans in collaboration with the interdisciplinary team (IDT), participants, and their families.
  • Manage acute and chronic medical conditions, coordinate specialty care as needed, and ensure all care aligns with the goals of PACE participants and their families.
  • Collaborate with community-based physicians and IDT in discharge planning from acute or long-term settings.
  • Help maintain care of participants in the home setting, making visits as dictated by participant need.
  • Participate in the care team's daily huddles, interdisciplinary meetings, and care plan reviews to ensure optimal participant outcomes.
  • Collaborate with Seen Health technology and operations teams to constantly innovate and improve care workflows, care quality, and decision making.
  • Offer compassionate, culturally competent, and evidence-based care to improve participant well-being and quality of life.

Qualifications

  • MD or DO required; active board certification in California for internal medicine, family medicine, geriatrics (preferred), complex care, home-based care, palliative care, or other related clinical area with continued certification throughout employment.
  • Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education.
  • Current, unrestricted medical license in the state of California; if not currently licensed, willingness to become licensed upon hire.
  • Reliable means of transportation.
  • Active state CDS Certification and DEA.
  • Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants.
  • Native proficiency in Chinese or Cantonese highly preferred, conversational fluency required.

Essential Job Duties & Responsibilities

  • Provides compassionate and high-quality patient care and implementation of evidence-based medical practices (70% clinical, 30% administrative/leadership).
  • Serve as a clinical leader within a multidisciplinary clinical environment to strategically improve health outcomes for empaneled patients in a coordinated manner.
  • Provide clinical direction for clinic support staff, such as health coaches, medical assistants, and front desk staff, to ensure excellent clinical care.
  • Participate and lead initiatives for improved patient health outcomes.
  • Work closely with business unit leaders who oversee the facilities and employees where our clinics are located and collaborate with them on how to improve health outcomes.
  • Develop and review protocols, training materials, and project-specific data.

  • See an average of 12 patients per day, with 100% outpatient care and NO obligations to be on medical call during evenings and weekends.
  • Lead or support onsite, mobile, and community-based efforts that aim to improve patient engagement, access, population health management, referral tracking, and clinical quality performance.
  • Serve as lead collaborating physician to a team of non-physician healthcare providers (physician assistants and nurse practitioners).

  • Travel to Lost Hills clinic, Delano clinic, and Mobile clinic is required.

Qualifications

  • Graduate from an accredited medical school (MD/DO).
