Overview

Under the direction of the Director of Nursing, the Licensed Practical Nurse (LPN) is responsible for providing quality care to residents of the assigned neighborhood. The LPN functions as a professional caregiver who is accountable for planning, implementing and evaluating all aspects of resident care, including physical, psychosocial and environmental needs.

Essential Functions

  • Documents according to nursing practice standards.
  • Administers medications accurately and in a timely fashion and provides treatments to residents according to physician orders.
  • Provides physical comfort and a safe environment.
  • Accurately carries out physician/provider orders in a timely fashion.
  • Initiates, implements, and evaluates care plans/MDS on an ongoing basis.
  • Accurately completes admission and discharge procedures.
  • Coordinates staff and resident care appropriately to meet resident needs.
  • Follows all policies and procedures, annual mandatory in-services, required meetings, and consistently meets other conditions of employment.
  • Performs other duties as assigned.

Education and Experience

  • Current CT LPN license is required.
  • 2 years of full-time clinical experience in nursing is required.
  • Must be able to speak, read, and write the English language in an understandable manner.
  • Comprehensive knowledge of geriatric nursing care, principles of nursing, and physical evaluation skills is preferred.
  • Basic knowledge of human relations skills necessary to deal effectively with the geriatric residents, their families, and CHHC staff.

1st: 7:00am-3:30pm 2nd: 3:00pm - 11:30pm 3rd: 11:00pm - 7:30am

Pay Rate

$35.00 p/h + shift differentials ($2.00 p/h weekday evenings, nights & weekend days, $3.00 p/h weekend evenings and weekend nights)

  • Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience.

Position Summary

The SSVF Program Manager provides strategic and operational leadership for Veterans Bridge Home's Supportive Services for Veteran Families (SSVF) program, ensuring high-quality, veteran-centered services that prevent and end homelessness among veterans and their families. Reporting to the Market Executive Director, this position is responsible for the day-to-day management of the SSVF grant, including program operations, staff supervision, compliance, quality assurance, and community partnerships. The Program Manager will work directly with the Compliance department and Finance team. The Program Manager ensures services are delivered in accordance with Department of Veterans Affairs (VA) regulations, Housing First principles, and Veterans Bridge Home policies while maintaining a strong focus on measurable outcomes and continuous improvement.

This position serves as the operational leader for the SSVF program by fostering a culture of accountability, collaboration, and innovation. Working closely with internal departments and community partners, the Program Manager develops strategies to improve housing stability, enhance program performance, strengthen partnerships, and ensure responsible stewardship of federal grant resources. The Program Manager contributes to Veterans Bridge Home's mission by connecting service members, veterans, and their families to the communities where they live and thrive. This is a grant-funded position.

Primary Responsibilities

Program Leadership

  • Provide operational leadership for the Supportive Services for Veteran Families (SSVF) program, ensuring services are delivered in accordance with VA regulations (38 CFR Part 62), grant requirements, organizational policies, and a Housing First, veteran-centered approach.
  • In collaboration with the Triangle Market Executive Director, assist with developing annual program goals, operational plans, and performance objectives aligned with organizational priorities.
  • Monitor program performance and implement continuous quality improvement initiatives to enhance veteran outcomes.
  • Lead program meetings, case consultations, and operational planning sessions to ensure consistent, high-quality service delivery.
  • Promote equitable, timely, and veteran-centered services across all assigned service areas.
  • Oversee delivery of both Rapid Re-Housing (RRH) and Homelessness Prevention (HP) services, ensuring the balance between service models aligns with SSVF Program Office priorities and available grant funding.
  • Direct coordinated outreach efforts to identify and engage eligible veterans who are experiencing or at imminent risk of homelessness.

Grant Administration & Operations

  • Oversee program intake, eligibility determinations, and referral processes to ensure timely, equitable, and consistent access to services.
  • Ensure eligibility determinations meet SSVF requirements, including veteran status, household income at or below 50% of Area Median Income (AMI), and literally homeless or imminent-risk status under the “but for” standard.
  • Partner collaboratively with the Compliance team to ensure adherence to grant requirements, support quality assurance activities, and prepare for monitoring visits and audits.
  • Maintain awareness of grant expenditures, fund utilization, and program capacity by collaborating with Finance to support operational planning and ensure services remain aligned with grant requirements.
  • Oversee the appropriate, compliant, and timely use of Temporary Financial Assistance (TFA), including rental assistance, security and utility deposits, utility payments, moving costs, and emergency housing assistance.
  • Monitor program documentation, reporting requirements, and operational performance while identifying opportunities for continuous improvement.
  • In conjunction with Compliance, complete, accurate, and timely submission of required reports and program data.

Staff Leadership

  • Supervise, coach, and develop assigned staff.
  • Foster a collaborative, inclusive, and mission-driven team culture.
  • Conduct regular supervision meetings and annual performance evaluations.
  • Support employee engagement, accountability, retention, and professional development.

Performance Management & Veteran Services

  • Monitor key performance indicators, including veteran households served, housing placements, housing retention, fund utilization, data quality, caseload productivity, and overall program outcomes.
  • Oversee timely and accurate participant data entry in the Homeless Management Information System (HMIS), including HUD Universal Data Elements and VA-specific data elements, and ensure grantee uploads to the SSVF HMIS Repository meet VA data quality standards.
  • Utilize organizational reporting tools, including Power BI, to evaluate program performance and identify trends.
  • Implement corrective actions and operational improvements when performance expectations are not met.
  • Support staff in resolving complex veteran situations while ensuring individualized, coordinated services that promote long-term housing stability and self-sufficiency.
  • Promote Housing First, trauma-informed care, and Veteran-centered service delivery throughout the program.

Community Partnerships

  • Build and maintain collaborative relationships with the Department of Veterans Affairs, Community Resource and Referral Center (CRRC), Continuum of Care partners, housing authorities, landlords, behavioral health providers, Veteran Service Organizations, and community nonprofits.
  • Ensure program participation in the local Continuum of Care (CoC) Coordinated Entry System for referrals, assessment, and prioritization of Veterans in need of housing assistance.
  • Identify opportunities to strengthen partnerships and expand resources available to Veterans.

Organizational Leadership

  • Support the development and implementation of organizational policies, standard operating procedures, and continuous improvement efforts.
  • Perform other duties as assigned.

Location

This role is hybrid, and the selected candidate must live in the Triangle Market area to support local activities and in-person needs. There will be a requirement to be in person to connect with veteran and supporting staff 2-3 days/week.


Salary and Benefits

  • Full-Time Hours: Schedule will be determined with the position supervisor to best support community events and client schedules. Schedule will include some nights and weekends. 
  • Compensation will be determined based on education and experience. Range is $55,000-$65,000

Are you passionate about overseeing and developing efficient shipment logistics for a wide range of varying accounts? If so, we have an opportunity that would allow you to provide a vital service for managing the coordination of Inbound or Outbound shipments, ensuring compliance with various government regulations for our Customs Brokerage Division.What Are You Going To Do?Oversee accounts - receive, review, and process complex or sensitive transactions for assigned base of accounts for all entry types, including the classification and valuation of goods. Process documents through the established system, obtaining release and other governmental agency releases as appropriate.Create and distribute written exception procedures for handling each account served by the team, including classifications, valuation rules, schedule of fees, correspondence, post entry process, and any other related information.Assists with overseeing the entry processing performance and existing customer service commitments to reflect established company databases and pre-load templates to reflect these procedures.Obtain certification for completing customer classification, as well as, other compliance specific classification as defined by company product standards.Assist and review work stream development of new and existing accounts, including reviewing all documentation and forms, maintaining communications with clients, reviewing their process on a regular basis, and maintaining client profiles and databases.Review and monitor all rejections, requests for information and other correspondence. Respond and assist with complex customer inquiries, including research and assistance with escalation on priority issues.Monitor shipments as they approach non-compliance based on customer control, event control, and transactional data count or control.Stay current of U.S. Customs regulations (Customs Brokers and the Customs clearance process); and classification and validation rules through self-study, approved related courses and seminars, and regular review of trade publications.What Are We Looking For?Education and Experience:High School Diploma or GED, Bachelor's Degree in Transportation, Logistics or Supply Chain Management preferred; Greater than six years related experience.Credentials:Professional certification may be required in some areas.Skills:Advanced proficiency in Microsoft Office, internet, web-based and job specific software applications; Ability to add, subtract, multiply and divide using units of U.S. currency, weights and volume and distance measurements.Characteristics:Responds promptly and professionally to customer needs; Ability to apply and assist with communicating laws, rules, and regulations in areas of assignment; Ability to write reports, business correspondence, and procedure manuals and gather and analyze documents skillfully.
