This position is located in Pottsville Pennsylvania in Schuylkill County. At our Claude A. Lorde location, there are 4 Physicians and 3 APCs, 6 MA's and 4 PSR's. The office is open 8a-5p with call being rotated with other Physician and APC's. Patient slot durations consist of 40 minutes for new, wellness/preventative for 45 and older, TOC, and medical clearance and 20 minutes for wellness/preventative under 45 years old, return patients and acute visits. Full time is considered 36 patient facing hours with part time and per diem being considered.

  • Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary
  • Uses all available resources in diagnosis and treatment, such as laboratory and others tests in an appropriate and cost efficient manner
  • Confers with other clinicians, patient s families and other support staff concerning treatment and care of patients when appropriate
  • Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care
  • Maintains accurate medical records and performs appropriate charting and follow up on patients
  • Participates in Medical & General staff meetings and appropriate clinical teams

About the Practice & Facility:
Serving a multi-ethnic community in the eastern part of New Haven,. All ten physicians on staff have clinical teaching faculty appointments to the Yale School of Medicine and all nurse practitioners hold faculty appointments to the Yale School of Nursing.
The facility provides care to over 10,000 patients of all ages in a patient population which is 62% Latino, 19% Black or African-American and 19% White. In addition to a strong emphasis on preventive care and wellness, the Center also offers excellent episodic care, continuity of care in the Hospital by Center physicians and 24-hour telephone access.
Special programs include an early literacy program, education and support for home daycare providers, group prenatal program, and wellness center. The Center also provides specialty medical clinicians on site including a gastro-enterologist, endocrinologist, nephrologist and ob-gyn. The staff comprises pediatricians, internists, advanced practice nurses (family and pediatric) and certified nurse midwives.
